Opting Out of Extended Healthcare

It has been brought to our attention a form titled “Benefit Payment Information During Leave” may have been provided to members who went on leave of absence without pay. Our understanding is this form is provided to members by the human resources department before an approved leave, and it allows members to opt out of the extended health care plan.

If you were asked to fill out this form and were allowed to opt out of the extended health care plan by doing so, be advised this is contrary to the collective agreement and puts you at risk of denial for reinstatement of coverage under the plan when returning from leave. If you opted out of the plan using this form, please send us an email at [email protected] for advice.

 

Web Based Course Development

It has been brought to our attention a form titled “Web Based Course Development Agreement” may have been provided to members when it comes to the transfer of copyright for web-based online course development.

This is not a form that complies with the collective agreement. Members are encouraged to contact CUASA immediately if they are requested to sign the form or if they have already signed it.
